Membership Commitment - All memberships require a six (6) month commitment.

Membership Cancellation Policy - Certain Revive Wellness and Recovery services are offered on an ongoing basis with a monthly membership. Users may request to have their membership canceled at any time by emailing hello@revivewellnessandrecovery.com. If you would like to request early cancellation of your membership, it will be considered on a case by case basis.  Your membership shall continue until the end of the existing membership period and shall terminate at the completion of that period. You shall not be charged after a cancellation. In the event that the Customer would like to cancel their membership they must inform Revive Wellness and Recovery of their intention to cancel their membership at least seven (7) days before the next scheduled payment in their billing cycle to avoid being charged for the following month. Revive Wellness and Recovery reserves the right to charge up to, but not exceeding one hundred and fifty dollars ($150) for early termination.

Gift Certificates - Gift certificate values do not expire and are transferable.

Package - Package values do not expire and are transferable.

Membership Refunds - All membership sales are final, and Revive Wellness and Recovery does not issue refunds. While no refunds will be given, all purchased but not yet redeemed IV’s will be available for one hundred and eighty (180) days following the cancellation of the membership.

Gift Certificate and Package Refunds - All gift certificate and package sales are final, and Revive Wellness and Recovery does not issue refunds. Funds for these items do not have an expiration date and can be transferred.
